The first thing that comes to mind is you are missing the Dr's NPI number,
NPI numbers are 100% requred and you can obtain them at
www.dmefree.com

another thought is that you are missing some portion of the Dr's address.
The doctors street address
city, state zip
are required and if missing will cause odd NPI errors
