Reminder: Bank Mergers and Electronic Funds Transfers (EFTs) National Government Services, the Jurisdiction B DME MAC would like to remind suppliers if your current bank merges with another bank, you must provide us with the updated bank routing information. This will ensure there is no interruption in your Medicare payments from National Government Services. In order to update this information you must complete the Electronic Funds Transfer Authorization Agreement form, which is the CMS-588 form. This form can be found at: http://www.cms.gov/cmsforms/downloads/CMS588.pdf After completing the CMS-588 form, you will need to submit the form and either an original voided blank check or a bank affidavit to the following address: National Government Services, Inc.Attention: Mail and Distribution Area – DMAC 588 formP.O. Box 7078Indianapolis, Indiana 46207-7078 Tips for submitting the CMS-588 form: Be sure to include an email address and/or fax number when submitting your CMS-588 form. This is the quickest way to address any problems that may arise with your application.Get a bank affidavit from your bank. Although your old checks with the routing and account number may still be valid, they are not valid for EFTs. When submitting the CMS-588 application, verify that your National Provider Identifier (NPI) and your Medicare legacy numbers are entered correctly.
